Transdisciplinary approach: A transdisciplinary approach goes a step further than just having an MDT (multidisciplinary team). It fosters a holistic management plan. Team members share knowledge, cross traditional role boundaries, and coordinate care seamlessly. This model is specifically designed to eliminate communication gaps, promote shared decision-making, and reduce patient and caregiver stress by minimising repeat visits, redundant assessments, and conflicting recommendations.
Condition-specific protocols: Tailored plans for neurological, cardiac, orthopaedic, trauma, chronic conditions and other medical problems that need well-structured recovery plans.
Integrated medical overseeing: Senior physiatrist and RMOs work with therapists, nurses and care-nurses to adjust care as recovery progresses.

The Twilight Grey team is led by Dr. Tripti Swami. Dr Swami is a seasoned clinician specialising in Physical Medicine and Rehabilitation (PM&R) with profound expertise in the management of complex neurological and musculoskeletal disorders. Her clinical acumen is grounded in years of experience at leading institutions such as NIMHANS, AIIMS Jodhpur, and CMC Vellore, where she has delivered high-quality rehabilitative care to patients with head injury, stroke, chronic pain, spasticity, and functional impairments.
Holding a Post-Doctoral Fellowship in Neurological Rehabilitation from NIMHANS, she combines advanced medical knowledge with a patient-centred approach to address the multifaceted challenges of recovery.
Through Twilight Grey Care, her mission is to establish uniform care protocols and continuously enhance transitional care services, transforming them into a well-organised sector comparable to hospitals. This effort significantly raises the quality of post-hospitalisation rehabilitation services across the country, aligning with India’s evolving healthcare priorities and quality assurance frameworks. Maj Rama Jaykumar is an armed forces healthcare veteran who ensures quality and efficiency in our operations. She has a strong clinical background in healthcare quality, with 20+ years of experience across various armed forces hospitals, mainly in intensive care units, healthcare administration, training and quality.
• Qualifications: B.Sc. Nursing, BASM, M.Sc. Nursing, MDBA, PGDHHM.
• Certified BLS, ACLS, ITLS trainer.
• Certified Healthcare Quality Implementer (CPQIH Advance).
