Twilight Grey Care
Recovering from challenging medical conditions requires personalised care that goes beyond the initial medical treatment. It demands structured, multidisciplinary care under the guidance of trained rehabilitation professionals. Twilight Grey Care’s transitional facility bridges the critical gap between hospital and home, ensuring seamless continuity of care. Early, specialised evidence-based rehabilitation proves essential for neurological conditions, particularly complex cases like disorders of consciousness, where timely interventions enhance emergence from impaired states and minimise complications.
Specialised Care
At Twilight Grey Care, we focus on care as the most important component of recovery.
To us, care is not a generic one-size-fits-all protocol. Every patient comes to us with a unique medical history, a specific condition and an individual pace of recovery. We design personalised care protocols after detailed assessments, making recovery structured, precise and specialised. These protocols are person-centric and trans-disciplinary, focusing on setting short-term and long-term goals collaboratively with the patient and the healthcare team. We assess each of our care staff and try to align them based on patient needs. By continuously assessing and adapting interventions, we create a dynamic and responsive rehabilitation process that maximises outcomes for our patients.
Our programmes are led by expert physiatrists (specialists in physical medicine and rehabilitation) who oversee personalised medical rehabilitation plans designed to reduce complications, prevent hospital readmissions and maximise recovery potential.
Our advanced therapies address each patient’s unique needs in a comprehensive multidisciplinary environment, supporting both patients and their families. Treatment doesn’t end at our discharge. We coach families through every step of post-discharge home care.

2. How is Twilight Grey different from a hospital or nursing home?
Unlike a hospital, we focus on recovery rather than acute treatment. Unlike a conventional nursing home, we offer comprehensive rehabilitation, which involves doctors trained in rehabilitation, skilled rehab nursing, and therapists trained in rehabilitation, such as physical therapists, occupational therapists, speech and language pathologists, and clinical psychologists, all working as a trans-disciplinary team under the supervision of a physiatrist.
Our care is personalised and protocol-driven to achieve the best possible functional outcomes and to support patients and their families in this journey.
